In order to prepare for Health Care Reform, a Health Care Delivery System must:

• Use population data
  
• Incorporate the entire provider network for aligned delivery:
  Identify members with interruption in their care plan (i.e. visits, medications, etc.)
  Emphasize education and follow-up
  Repeatedly re-stratify all membership monthly
  Integrate behavioral, physical, and social issues by member
• Aggressively self-measure its performance
• Robustly match its current and future targets with government-defined measures of performance
• Ambitiously seek connections with global membership
